Kuuga Ultimate Form (Black Form)
Perubahan ini adalah gabungan dari rising dragon form, rising mighty form, rising titan form, rising pegasus form, dan amazing mighty form dan menghasilkan Ultimate form - red eyes. Ultimate Kuuga dapat mengeluarkan senjata rising tetapi warna senjata akan berwarna hitam dan core belt akan berwarna hitam. Jika Yuusuke mengeluarkan kekuatan penuh dari ultimate kuuga dan tidak bisa dikendalikan maka form ini akan lepas kendali - black eyes.

Perubahan yg sangat berbahaya jika digunakan bahkan taruhannya seluruh dunia. Yuusuke harus menguasai perubahan Ultimate Kuuga - red eyes. Perubahan ini muncul pertama kali di Kamen Rider Decade Movie : All Riders VS DaiShocker. Bahkan Yuusukepun tidak dapat mengendalikan form ini - black eyes. Jika Yuusuke ingin memakai form ini dia harus mendapat bantuan dari Decade K-Touch.

The transition was rough on Shōichi's body, as he was virtually incapacitated during the emergence of this new power. But as they manifest, the nature of the power became clear; and it is one not easily controlled.

Blaster Form
Blaster Form (ブラスターフォーム Burasutā Fōmu?) is the most powerful form of the Faiz Gear, the amount of Photon Blood that the Faiz Gear produces increases to the extreme where it overflows from the Photon Streams and spread itself over the Sol Foam and the Sol Metal, changing their color to red.
Faiz also receives a new, more powerful, version of the Full Metal Lung, with a backpack-like device attached to it called Photon Field Floater (フォトンフィールドフローター Foton Fīrudo Furōtā?), which releases constant waves of Photon energy to give Faiz Blaster flying capacities.
The Photon Field Floater, however, can be used as a weapon by swiveling the exhaust ports onto the shoulders, which are then dubbed the Bloody Cannons (ブラッディーキャノン Buraddī Kyanon?). The shots fired from the Bloody Cannons are focused Photon energy.
In Blaster Form, Faiz inputs all his codes directly on the Faiz Blaster.

Blade King Evolution
The King Form is the ultimate combat extension of the Rider System, which only Blade can obtain. Using the Rouse Absorber, this transformation is initiated using the ♠K's "EVOLUTION KING" effect in conjunction with the ♠Q's "ABSORB QUEEN" effect. The King Form addresses many issues with Blade's combat capabilities, such as its defensive capacities, with a complete armor overhaul. This new heavy armored design is gold-plated and is increasing resilient to damage, but at the sacrifice of agility. Nevertheless, with all these improvements, Blade's King Form suffers from one glaring flaw: Kazuma Kenzaki. Initial specs intended only for the Blade Rider System to extend its combat capacities with the ♠K: "Evolution Caucasus", but due to Kenzaki's unusually high fusion co-efficiency, the system exceeded its perimeters and exposed a disastrous glitch in the overall Rider System. In the case of individuals like Kenzaki, activating the King Form extension initiates the fusion of, not just one, but the DNA of ALL 13 sealed Undead associated with a particular card-suit. With extended activity in this form, the subject's fusion co-efficiency would raise exponentially, ultimately mutating the human being into a Joker-type Undead. To that end, use on Kenzaki's part, was strictly cautioned and advised only as a last resort.

DoGaBaKi Emperor Form
DoGaBaKi Emperor Form (ドガバキエンペラーフォーム Dogabaki Enperā Fōmu) is actually a true final form seen the Adventure Battle DVD, when Kiva accesses Emperor Form while in DoGaBaKi Form. In this form, Kiva can use all three Arms Monster weapons and execute the DoGaBaKi Emperor Break.


Fangire Emperor Fom
When Wataru's Fangire blood is awakened by the Swallowtail Fangire he temporarily became a Fangire version of Emperor Form gaining the Fangire's signature stained glass appearance and becoming a berserker attacking friend or foe. This caused Wataru to go into isolation until encouraged to fight again.


Emperor Form (Golden Kiva)
Emperor Form (エンペラーフォーム, Enperā Fōmu) is the true form of Kiva, referred to as the Golden Kiva (黄金のキバ, Ōgon no Kiba) and the most powerful form at Wataru's disposal. The fifth form accessed, the first emergence of Emperor Form comes about from the reaction to Wataru's rage from the harm coming to Mio, his girlfriend, which summoned the Demon Imperial Dragon Tatsulot to awaken the form. By pulling the switch on Tatsulot's head, Wataru can activate Tatsulot's Imperial Slot (インペリアルスロット, Inperiaru Surotto) and activate one of several "Fever" finishing attacks through a roulette system. With this form, Kiva can destroy both the body and the soul of a Fangire.
Because of this Wataru has gradually abandoned using his other forms in favor of Emperor Form. Emperor Form initially appeared in promotions for Kamen Rider Kiva: King of the Castle in the Demon World at the Tokyo Toy Show 2008.
On the album SUPERNOVA by TETRA-FANG, Emperor Form is referred to as "Empire form."

Hikawa tengah menyelidiki kasus tsb, ketika "Agito" dan unknown berjeis anjing menyerangnya.
Exceed Gills dan Another Agito muncul dan membantu, tapi dapat dikalahkan oleh "Agito".
Tak lama, Tsugami Shouichi datang.

Rupanya, "Agito" yang bersama dengan Dog Lord adalah seorang psychic bernama Maguchi Shouichi yang menjadi gila ketika anjing peliharaannya mati. Ia rupanya mengira Dog Lord adalah anjing peliharaannya.

Agito cs mengalahkan dan menghancurkan Dog Lord tsb, sehingga Maguchi mengamuk dan berubah menjadi...Mirage Agito.
Agito berubah menjadi Shinning form dan mencoba menggunakan Shinning Kick padanya, tapi Maguchi pergi, kembali ke wujud manusia dan menyerahkan dirinya.

Kamen Rider Diend

Complete Form
The final film is Episode Yellow: Treasure de End Pirates. It is to be released on June 19; advertising for the film currently focuses on the cyan-colored Kamen Rider Diend who only has minor yellow accents on his suit and a yellow Diend Kamen Ride card. The story features Daiki Kaito traveling to the World of Den-O again and his involvement in the theft of the DenLiner and Rider Pass, the result of being possessed by the Spider Imagin, previously announced on the blog of Kimito Totani who reprises his role as Daiki Kaito from Kamen Rider Decade for the film. Kamen Rider Diend's Complete Form will also be seen in the film. He is chased after by Reiji Kurosaki as Kamen Rider G Den-O for his crimes against the timeline.
